WHY IS THE DMV SUSPENDING MY LICENSE AFTER AN ACCIDENT?!?!

We sometimes come across this question and it usually has to do with an “SR-1”. The SR-1 is a form that the DMV requires a driver to complete after an accident, regardless of who was at fault, when:
Damage to property exceeded $1,000
There was an injury or fatality involved
Most accidents fall into one of these categories.
What Happens If I Don't File It?
If you fail to complete the SR-1 and the DMV learns about the accident (through a police report or the other driver filing the form and naming you), the DMV could suspend your driver's license and require you to obtain an SR-22 certificate with your car insurance.
How to File the SR-1
It's actually quite easy to complete the form. Your own insurance company will often send you a copy by regular mail, or you can access it directly through the DMV's online portal. You can even complete the SR1 form electronically. This is probably the best way, it's fast, and you can immediately print out a confirmation for your records.
